Welcome to Staveleigh Medical Centre which is located close to the centre of Stalybridge.
We are based in a purpose built medical centre. The surgery provides parking facilities for Doctors and staff only due to the limited space available. There are various parking facilities near to the practice. Access to the building is through automated doors at the front of the building and there is provision of a ramp facility for disabled service users.
We have three Principal Doctors and a Salaried Doctor and a full compliment of ancillary staff looking after the well-being of our patients. Our Doctors and staff aim to provide a highly professional service but one that is offered in an accessible and friendly way. We have an excellent team of nursing staff who work alongside our Doctors who provide a wide range of clinics including chronic disease management for our asthmatic and diabetic patients, weight management, smoking cessation and various other general health checks.
We hope you enjoy having a look around the site and familiarising yourself with some of the online features such as ordering a repeat prescription. Patients and the Summary Care Record
The NHS in England is using an electronic record called the Summary Care Record (SCR) to support patient care.
The Summary Care Record is a copy of key information from your GP record. It provides authorised healthcare staff with faster, secure access to essential information about you when you need unplanned care or when your GP practice is closed.
Summary Care Records improve the safety and quality of your care.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything. If you have any concerns or do not wish to have a summary care record, please speak to practice staff or download a copy of the leaflet “Patients and the summary care record”. You can complete an opt out form at the practice and hand it into the reception staff to process.
